Are all voice VLANs trusted on the MXE in MSL?  The IP phone would need to communicate directly with the MXE due to legacy PRI.  Make sure they are trusted  on the virtual too, but you are correct that wouldn't be part of the call for IP phone over PRI.  Is there an E2T card on the MXE?  Did you reuse any hardware from the original MXE on the swap?  Bad DSP?

If somebody migrates from 8 to 9 and they use an account that is not the system account to run the migration, the MSL password for admin becomes that users password.  You will have to have that person login as admin to MSL and change the password.  I don't know if this is documented in any KMS, but I've seen it first hand more times than I care to remember.  Then you have to figure out which co-worker did the upgrade to get access back.

Embedded voicemail will play the main AA if you send digits to the VM hunt group that don't match a mailbox.  The first extension that answers a public call is what usually gets sent to voicemail regardless of the amount of transfers.  However, if you use interflow or a nametag hunt group as the next DN in your routing those digits get sent to VM.  Sounds like your ACD group 1111 is being presented to VM still, but internally you dial 1112 to skip it so it works.  I'd just create a trusted extension with a mailbox and make it the interflow of group 1111 so it goes there when the timer expires.  When nobody is logged in it will go there automatically if you set it as the unavailable destination.  Then you just put the MWI for this extension on a few agents phones to access a group mailbox.

I have a version 5 and 6.2 in my lab.  I did a factory reset from the front panel on both.  Both used admin / itpassw after the reset.  However, if you reboot the controller before logging in, changing the password, and saving to the system flash then the old DB and login come back because you never saved anything to the card, its only in memory that the reset happened.

MBG / SRC using an MiVB needs a cert involved with the recording application and the MBG.  Have you made sure that exists?  That was a requirement back with MiVCR and also still now with MIR and any application using an SRC that I've ever known.  It decodes the MiNet audio.  You can have all the rules you want to record, but with out that it will never work.

I have done this before.  Break the internet connection out to the AMC on all of these controllers and reuse the ARIDs on the new VMs.  You need to clear the hardware IDs before you can use them again.  You get 30 days until the old system goes into critical alarm and basically shuts down.  You might have to make some AMC changes on your ULM or DLM depending on how that was built originally.  Move any licenses off of the physical ARIDs into the VM ARID before you deactivate them in the AMC to get the most licenses possible.  This gives you 30 days to log into the old systems for DB gathering and program the new ones.  Should make for a super smooth cut over.

Over the phones or a PA?  Need more info to help.  If over the phone setup a FAC for direct page and build a page group.  Its in the help / task guide on the MiVB.  If its PA you need a SIP device like an Algo or you can butcher a Mitel IP phone if you have an extra or beat up set.  Connect the output from the Algo or cut the speaker leads from the IP phone to get audio to your amp.  Mitel phone still needs to be in a page group. SIP like an Algo will be a direct extension dial. Mind you this is extremely generic descriptions.  Not knowing what you are doing or what equipment is involved makes it difficult to answer.

It looks like you are sending the call from this controller to PBX2.  This doesn't appear to be the actual outbound call.  Do you have the COS / COR allowed to external forward on the other PBX?  Trunk to Transfer enabled on trunks?  Two B Channel transfer allowed on trunks? Seems like a COS / COR issue if it works internally when dialed but not for a public call.

You need special software to change the programming from the Mitel downloads page, which is located behind a partner only login portal.  If you don't know the password to the system you'd have to have a Mitel partner open a ticket for a password recovery which costs a few hundred dollars.  With only 4 phones it might be cheaper to factory default the system and have somebody just reprogram it real quick.  A system that size should be 30 minutes or so to reconfigure from scratch.

Disable the trunk in MiVB Network Elements then disable the trunks on the MBG also, then turn both back on should do it then.  Generally the Network Elements does it by itself.  A full reboot is a red flag something isn't setup right.

Should be able to go into Network Elements and disable then re-enable the trunk instead of a full reboot.  That would force a re-register.  Do you have an MBG?  MiVB is not supposed to be connected directly to public SIP trunks via NAT.  Documentation shows that is not supported.  If you have a local private IP hand off for SIP then that should be OK.

Using bootable USB?  Build the bootable drive using Rufus in DD mode.  Something about the image bring corrupted if not built in DD mode.  I have had this happen many times the last few years to my techs.  If you burn MSL to a DVD this doesn't happen.
