Keep getting One time username failed on switch. Does nayone know why this I am getting this error?

Can you please provide information on the type of PBX, and the exact error? A snapshot of the error would be good too.

Sorry stevet. I've never worked with HTML Toolkit Uploader. My company's security policy is very strict and user's are only allowed the "basic" use of phone.
