Employee phone change

I have an employee that has an extension tied to her name with the mac address of her mitel 5360 phone.
She is moving and the phone will not be going with her.  Instead, we will be installing the UnifiedCommunicatorAdvanced.msi and setting her up as a teleworker.  How do I remove the phone from her name, but keep her same extension as a teleworker?

You need to renumber the 5360 phone to something else.
Delete the name or rename it.
Then you'll need to build a phone with her old extension number for the TW phone.

Got the user setup with a softphone, but is unable to dial out and not able to receive calls.
Says micallab is unable to dial out.

The only thing I can figure out is the extension is 1733 I'm not able to program service level from full to multi-device.  When I make the change and press save get the error invalid service level change.

Answer to the invalid service level error:
Not allowed.
You can go higher in service levels but not downgrade.
- i.e.: Ip Device only -> Multidevice -> Full

Under the new User and Services form:
- locate the user by last name
- change the directory number of the old service to free it up
- add the new service with the right number
- make that the primary phone service
- disassociate the old device and do whatever you would like with it
This is not so simple if you have MiCollab with SDS enabled, because it is recommended to do this in MiCollab.

Dialing issue with MiCollab client.
- back to basics, dial another extension number
- COS: Trunks- Public Network Access via DPNSS

This is our 2nd teleworker.  The first teleworker using the same uca software is able to dial out and receive calls.
This recent user at x1733 is unable to make or receive calls with uca.  Screen shot attached.

Found x1733 wasn't added to the multi-device user group.  Added it along with 17*33 and she is still not able to dial out.  I'm at a loss.

In micollab changed from premium to UCC Premium and added UCC Premium to bundle under General tab.  Now user can dial out, but she is not able to receive calls.  Desk phone shows a red x.
