Default FAC import spreadsheet

Does anyone have a copy of the default FAC import CSV spreadsheet that MiCW creates? Or an export from a newly configured system that MiCW did that hasn't been changed yet? Preferably from MCD 7?

Hope it helps someone. Now if I could just get all my default COS import sheets back, they were just the default ones built by MiCW...

I kept them on my laptop and a flash drive... HD crashed and flash drive is no where to be found. :/

Maybe if I get time I'll spin up a new OVA, run the MiCW, and do fresh exports... But that might be a while.

I just got back into town and saw this post.
Sorry I'm late with it.

I've never been happy with their "recommended" codes.  I prefer to use a "*" to activate and a "#" to cancel.
It makes it just a little easier to remember IMHO.   
Such as
   Call Forward Always On = *65     
   Call Forward Always Cancel = #65
