XML Error on user pages - possibly related to system speed dial

Hi,
I am getting a strange error on user login pages.  When a user logs in, they get an error message on the page that says:

[2015-02-20T10:29:06] error parsing forwarding XML - object invalid: {type: 2, destination: 1, number: }

They are completely unable to change the button programming on their phones; it tries to load, but just pops up this error message and never loads the page.  Tried repairing the database, it said that it had over the limit of 1000 errors on it.  So I reloaded a backup, and this worked correctly.  However, the clients had added system speed dials since the backup, and as soon as they added the system speed dials back in, this error reoccurred. 

Thoughts on this?

What version system is it on?
