5212dm in SIP mode on MCD6 - how?

Trying to set up a 5212dm in SIP mode on an MCD6 to troubleshoot issues with a ClearOne VH20 connection. I've got the set in SIP mode and pulling date and time, but I can't seem to figure out the rest of the settings. My previous SIP endpoint configuration experience is only with UC360's and XLite, and this doesn't look much like either of those. Anyone have a guide?

I didn't think it'd be as easy as browsing to the IP of the set, but I tried it and there's my settings.
