Creating a "support package" for Mitel Support

Guys,
Mitel support have been asking for a "support package" when logging faults.

I don't remember ever being shown how to do this on the training and a quick Google and scan through the manuals did not tell me how to do this.

When i figured it out, not difficult really, i thought i would share with the community.
