B-Channel not disconnecting

I'm seeing issues where a b-channel gets locked up. I can't access it for outgoing, get busy. In coming callers hear silence then busy and reception phone rings but no connection is made. The carrier says not getting response from our system. Error log shows:

-01:284- 11:29 11-12 *** default_trs_t::ti_setup_ind - We received a setup request for a new call while we were on a call.
-01:285- 11:30 11-12 M3109 INF EG CP Network Trunk ID(1:'94000') Did Not Receive a Release Ack For TI Call ID (0x0111)
-01:286- 11:30 11-12 M2002 INF HW CP [3.1.1]: Off-Line From B-Channel Trunk Device
-01:287- 11:30 11-12 M2002 INF HW CP [3.1.1]: On-Line From B-Channel Trunk Device

Resetting T1/PRI circuit clears issue for awhile or deleting channel and rebuilding it clears it for awhile.
Any clues to resolving this?

ISDN Switch Type?
I had one setup where the telco said they had the PRI set to DMS 100.  I had my Mitel 5000 set to DMS 100 and channels would just start locking up.  Changed the ISDN protocol on the 5000 to National ISDN-2 and everything worked fine.

I did ask the carrier if National ISDN 2 was the correct switch type and they confirmed.

JerryM,

Delete the T-1 and recreate it. If you have a Dual T-1 card then use the other port and if it works move it back. This has happened a couple of times and the DB Test does not find a problem.
