If I forward extension A to extension B, call remains ringing on extension B en does not follow it's routing. Do I miss a cos setting?

Are you doing the forwarding via ESM or at the set?

Are you doing true forwarding, or a reroute via Call Routing? Remember the Golden Rule of Mitel, you can't reroute a reroute...

I do a forward by a feature access code on the set. Not via ESM or rerouting

Here's a general rule of thumb:
You cannot re-route a re-routed call.   If a call follows a reroute (using the call routing forms) then it will not follow the reroute of the call you sent it to.

You can however, forward a call from one phone to another to another and to another (up to 10 if I remember correctly) via the call forwarding FACs.

Value is from 2-10 in the 'System Options'  Form 'Number of Forward Hops'

If you are on a 3300, got o User and device - Advanced - Call forwarding form.
You will see all forwarding settings regardless of the way it was set.
If the destination number is not set, the forward won't activate.
If you have a Mitel multiline set, just program a key on the phone. Gives a visual indication when active.
