SX-200 ICP caller ID problem

the problem is that there is no Caller ID displayed on incoming calls. All calls go to voicemail auto attendant. In this scenario the call goes to a 5340 IP phone.
The caller dials an extension while listening to the greeting. The call rings the phone and while the phone is ringing, the extension number of the voicemail port that is transferring the call, is displayed. When answered, the display shows either the phone number of the caller or the name of the caller. Depends on whether COS option 503 is enabled or not. If the person answers the call and transfers the call to another 5340 phone the name and number are both displayed while ringing and after answered.

Why isn't the name and number displayed when the call is transferred from the auto attendant? I verified that COS options 502 and 613 are enabled on the COS for the voicemail ports and the extension being transferred to.
