Speaker Button

I have an extension where the speaker doesn't work and have tried 3 different handsets.
I don't see any system or extension flags to turn off speaker and the keymap has it set correctly.
Is there something I am missing?

Phone is in Headset mode?

317 is the (default) code to toggle it on and off.

Yes that was it. Odd it didn't show that within DB Programming.

There is a flag... in System > Devices & Feature Codes > Phones > xxxx > Flags, it is called Headset On/Off and should be No under normal circumstances. If you are using a wired headset it maybe set to Yes and the speaker button is used to toggle the headset on and off.
