Intermittent issues when transferring calls

Hello everyone

One of our users (on MiCollab 9.4.109) is getting some weird intermittent errors when trying to transfer calls to another user (happens about 7-8 times a day) - the problem is after the call is transferred, the user continues to hear the conversation, and MiCollab comes up with "TransferCall request failed", as per the screenshot.

This error can't be dismissed and calls can't be taken again until MiCollab is forced closed and opened again. I've tried searching the logs under %appdata%\Mitel Networks Corporation\MiCollab\log , but I'm not too sure where to start looking, I can't find anything to suggest anything is failing.

Has anyone had this problem before or maybe got any advice on what perhaps to check? We have replaced the user's headset as someone suggested but it still appears to be failing.
