Can't get a 3 way party working on a PolyCom IP 6000

This is frustrating!  I can have two people on hold but when I hit the Conference soft key to bring them together, I get the dial tone.  I can't get 2 other phones to conference in with my IP 6000.

I've tried messing with the Line Settings and all that. 

Could someone please point me which setting I need to change to get this to work?

Will it conference the lines together if you use the Mitel FAC?

Does the COS allow a Conference Call in the Advanced tab?

Did you program a second Multicall appearance of the first line on the second key in key programming?
