Is this a statement or a request for help?

Don't know about the 5000 but on the 3300 you must have ports 3999 and 3998 open from MBG to phone and MBG to 3300.
Then, if you're running the latest version of MBG then you may need to be running the latest version of 5k since (at least on the 3300) SSH support was only added recently and non-ssh was removed for html app.

It is actually a request for help.  I added the teleworker on site like the instructions tell you, and it comes up with the extension like it should.  You put all the info statically in the phone with the IP settings and take it off site.  As the phone boots up, it comes up temporarly with the extension like it should, and a screen pops up saying dowloading html that is as far as it gets.

Was this phone ever working on site?   or was this a brand new phone?

Is the phone type set to NAT rather than native under endpoint IP settings?

I'm struggling with the exact same thing with a phone now.
Phone was working fine, then I executed a htmlappsupgrade command.  (silly me)
This broke it.
It appears to be an incompatibility between the version of 3300 I'm running (9.0) and the MBG version (latest).
I'm working with Mitel to try and resolve.   It's possible that my only solution is to swap the phone out for a non 53xx phone.
We'll see.
I'm thinking that there is a file on the MBG that I may be able to remove that may resolve.  - Now I'm in hacker mode again.  I'll let you know if I come up with a solution.

I finally cried 'uncle' on this one.
I'm sending the user a 5220 phone so I can get her up and working.
I can swap it back out after their 3300 is upgraded.
