Line or Call Swapping MiVoice Business 7.2

Has anyone come across issues the MiVoice Business v7.2 where 2 parties will swap calls?  We have been experiencing issues where employees are on the phone with people offsite via PRI calls and for no apparent reason the calls will swap.

Is the PRI connected to  Dual T1 Framer card installed in the 3300 controller or something else?
What type of phones?  It seems to me that years and years ago I had experienced something similar with digital phones but never with IP phones.

The PRI is connected to T1/E1 Card and the phones are Mitel 5224 and 5324 IP phones.

This has happened at multiple locations all running the same version.

Anything unusual in the maintenance logs at the same time these swaps happen?

Nothing is showing unusual in the maintenance logs at or around the times these occur.

This is the part that is really unusual.  What is strange is if I perform scheduled reboots of these systems say weekly the problem is not seen, but as soon as you stop this and let it go a couple of weeks the issue is seen again.

Our company does not like to do reboots because we run 24x7 and this causes interruption of services.

Route optimization problem?  You may try to disable it to make sur.
