Call Issues - Hold Music, Disconnect, Loud constant sound

We have a Mitel IP Phone system. We are having lots of issues with dropped calls, hard to hear, on both ends sometimes.

What made me post this is I was at home talking to someone at work.

Then all of a sudden a long constant noise came through the phone, couldn't hear her anymore, it put me on hold by itself, heard the hold music, then randomly did it a few more times, while I was still listening to this weirdness, she called me on another line, said she never put me on hold, I was on speakerphone the whole time.

So how does it randomly put someone on hold? And what is the loud constant sound?

Mitel makes several different phone systems, which one do you have? What do the connections between the phones and the servers look like? Audio quality issues are usually related to the underlying network and not the Mitel hardware/software itself.
