what router ports need to be open when connecting ip sets via vlan ?

Why would the ports change by vlan?

Have you checked the engineering guidelines?

There are about 9 pages of ports listed schematically in the Engineering Guidelines.
You should figure out what path (eg, phone<---->phone; phone<---->controller) involves passing through any kind of filtering (eg firewall or router with access lists) and check the ports in the guide.

Remember you cannot do NAT translation either... the MiVB does not support it.
