Mitel MiVoice Business/MCD/3300 / Paging on EX controller failing

I'm trying to do paging on an EX controller through a Viking FXI-1A paging interface and involving a Viking FBI-1A feedback eliminator. On the EX, I've set up FXO port 4, done the configuration including running the config script for FXO, and wired to the Viking FXI ports 1-2. I have ARS digits 360*82 and route pointing to the FXO port.
On Mitel KMS, I found and configured the underlying EX based on the article SO4543 (https://mitel.custhelp.com/app/answers/answer_view/a_id/1012592/loc/en_US). My transform rule is for " 360(.*) ".
When I dial 360*82 from a phone, I do get a connection in that the Viking light turns blue and the call timer on the extension starts to increment. When I speak into the phone, I intermittently hear my own voice echo back to me and nothing comes out the paging system. When the call timer hits ten seconds, the call disconnects.
I've tried removing the feedback eliminator, tried swapping the wires, etc. Nothing seems to change this frustrating behavior/failure. Has anyone had similar circumstances that they debugged?
