Mitel MiVoice Business/MCD/3300 / Calls unexpectedly go immediately into voice mail instead of ringing phone

The phone is not forwarded. Phone is set to ring 4-5 times before voice mail picks up. This happens intermittently with internal calls and external calls. Has anyone else had this experience? We have had it happen with our main number and several other extensions. We have a 3300 running software version 7.
