Mitel MiVoice Business/MCD/3300 / Audio issue when on a conference call

We have an issue that has come up over the last few weeks, where a caller (Either on a Mitel 5330e or Mitel UC360 phone) is on a conference call, the sound cuts out. The phone is connected, but they cannot hear, or be heard. Then it either comes back, or they have to dial back in to get connected correctly.
It occurs with incoming, and out going calls.
It could last 5 or 10 seconds, then the audio returns.
How do you trouble shoot this sort of thing? Is there a log file that may reveal the problem?
