I would check what number she is sending outbound and talk with the carrier sounds like they might have that set in their switch. Try a cpn substitution to a working number and see if it changes would be an easy way to tell if it is a carrier issue.

There are a lot of ways to skin a cat. I get the licensing reason but from a setup a hub and spoke prospective is a  cleaner and is easier to troubleshoot send me your config I might be able to point out where the issue is as I have my theory. I also look at it as another point of failure to voicemail going thru another server just my curiousity that killed the cat.

Yes some sites are still using old versions however as releases are increased support goes away for older releases or become costly. A highlight of progression from 9.0 is 9.0 --> MCD 4.0 --> MCD 4.1 --> MCD 4.2 and soon to be MCD 5.0.

9.0 UR3 is approximately a year old and 9 ish releases behind the current. That is a rough give or take.

There is already an ongoing discussion on the forums about this. To anyone working with SIP use wireshark to get accurate captures. I have this working with seperate trunks just fine and I see that as a better setup than traversing lync for all rerouting. However if you provide a wireshark of a bad sip call and your sip peer profile and incoming digit assignment we could probably solve both problems fairly easily. Why not have two sets of trunks?

Yes there is a guide available on MOL that lays out the setup in detail. There is no need to fill in proxy settings if you are not using a proxy. The way it sounds you have it setup correctly now with removing those settings.

Yes RCC has some odd behaviors as it is meant to tie into a Mitel PBX through the use of LBG. I have still not got a working LBG config but I am leaning towards internal issues with that. This also removes video calling due to Microsoft brilliance.

As far as Exchange goes I have a second set of trunks because I also reroute regular Mitel phones to Exchange voicemail. I would assume you could have it come off mediation by doing some number routing.

Depends on number of pbx entries and such and VAR cost and markup. I would check with a local VAR for cost.

It depends on how and to what point you are wanting integration. You can use lbg for more integration or just SIP for simple calling. With lbg you can click to dial however it does not support acd, multicalls, or conference calls. Main issue is windows hanging and having to manually close them.

Not easily without Mitels log viewer the is internal to Mitel.

1.) Make sure that you are using the same formatting in your Lync extension programming +4222 and +4567.

2.) What is the extension you are trying to call? You need to have it listed in your incoming assignment form. (ie, 1*, 2*, 3*)

3.) I believe the setup that I got from Mitel was different then the options you have set but I have further modified it to fix some 180 issues as well as simultaenous ringing.

     a.) Force Sending SDP in initial invite message - Yes
     b.) Precent the use of IP Address 0.0.0.0 in SDP Messages - Yes
     c.) Suppress User of SDP Inactive Media Streams - Yes
     d.) Disable Reliable Provisional Responses - Yes
     e.) Enable sending + for E164 numbers - Yes
     f.) Use P-asserted Identity header - Yes
     g.) Session Timer - 0

All others are set to no and times are all default.

Yes I was going to mention that make sure that is the only thing that uses that route as you will get notification on all calls. The advantage is you can remove calling restrictions as well as have emergency notification of these calls. If the route is shared you will get that notification on any call that passes the route.

You then should look at List 11 for your route info.
Then on the Route assignment Route A change type to Emergency.

I would say to probably get a packet capture to look at and see if there is any missignaling or audio ports going to odd locations as a starter.

Not sure I just typically do Logsys Read SMDR All then Ctrl A, Ctrl C, and paste it into notepad for cleanup or searching.
