There is no definite guide on SIP as SIP is an open protocol it varies by vendor. That is starting to narrow down as time goes on but it is still very visible if you have a problem you encounter post it and we can help you trough it. I have spent thousands of hours on SIP so I have seen about everything I think but then there is always something new.

Not more SIP problems 

I would...

1.) Start with a trace of the call probably monitor the port of the exchange server and see what is happening.
2.) The DND issues that I have seen and is very similar to your issue can be resolved by a couple means...
     A.) Make sure DPNSS/QSIG Diversion Enabled is no in system options.
     B.) Make sure public network to public network is enabled in the trunks COS.
     C.) Most important of all set Suppress Simulated CCM after ISDN progress to no in the trunks COS.

This should fix it if not remember SIP = Stiill In Progress.

Yes change to a speedcall key. A DSS/BLF is Direct Station Select/Busy Lamp Field or essentially a speedcall with status such as when they are on the phone, forwarded, dnd.

Well voice encryption is disabled through system options, call control is another ball of wax.

Yes I would recommend at least upgrading to 4.0 they have fixed a lot of the mid call signaling issues since 9.0.

What if you overflow to a phantom extension such as analog or a multicall on a phone to test? Just throwing ideas out there I will have to test it to tell you exactly when I get some time.

Sorry that should have said not but on a quick look at the guide is says press the phone key then long press the book looking key and volume settings should appear then press same key to adjust volume.
