Also a long time for me, However from memory one of the files in the Licensing folder holds the key, on the server look under program files for possibly a new heights (New Heights were the software developers at the time) folder or Mitel folder within there somewhere there is a folder called licensing and within this there are files you can open with a text editor.
It may also have been in the Sync folder?

Sorry to be vague but havnt seen one for a while.

look in C:\Users\<username>\AppData\Roaming\Mitel\ (windows 7) you will find a  text file called UCC, this is the log file and may provide more detail on the failure

You cant add manually but if you tick the box sync dynamic extensions only is will not import anything other than PRG info

No there is no softphone as yet, you can use office link instead, There is extra port forearding required for Chat, check the engineering guidelines.

A requeue happens when the agent is auto logged out so any report that shows logout time will tell you when the requeue happened, Alternatively use the Auditor.

The administrator guide gives you details on creating a transform script for install. If you do a search there was another thread a couple of months ago that covered it off from pretty much all angles.

It will be the local network option as stated by Blue, Not sure where you got the phones were on the same subnet m8 as I read it as they are on the other two mentioned.
Anyway Adding All your subnets in local networks put a rule in the Firewall on the MSL to allow DTMF to work.

Without going in to things in depth, You have a short abandoned time programmed against your path, As long as the user hangs up before this is reached it will not show against abandoned calls

Never tried it but assumed you just create an additional enterprise
