When an external caller calls someone that has their phone forwarded they do not hear ringing even though the destination phone is ringing. For example, I forward my mitel phone to my cell phone and when someone outside calls my DiD they do not hear ringing but the destination cell phone rings. If I try calling the forwarded phone from another mitel phone I can hear ringing. It seems to be a trunk COS, but I can't figure it out.

What type of trunks do you have?
Sounds like a SIP issues.

if the trunk is SIP trunk, try to set the option "Disable Reliable Provisional Responses" to Yes in the SIP peer profile.

We have a device that converts the trunk from SIP to PRI, so it's not a true SIP trunk. If I change the COS setting "Suppress Simulated CCM after ISDN Progress" from NO to YES for my IP trunk I can reproduce the issue where the caller on a mitel phone does not hear ringing. When its set back to NO the caller hears ringing again. I have tried adjusting this setting to NO for the PRI and SIP trunk COS but it does not seem to make a difference.

I'm confused, do you have any SIP trunk configured in SIP Peer Profile or not?

The only SIP trunk we have configured is for our conferencing server. It is a PRI trunk.

I would raise this with your provider/carrier.
It could be an issue on their end.
