Title: All phones rang simultaneously, Went to pick up got "Extension is busy"

All phones ringing at the same time, the phone was picked up and got Extension is busy, Any ideas?

My guess would be that another device got the line before the user did.

That was my initial first thought, but there was only one person in the building? Ghost??  :P

You could go back and look at the smdr to see if you could see anything.
