I have one extension that records every call made as if the record a call application is being activated. I've done no programming to this extension and swapped out the phone set, same result. I just told the user he's got the "over 50" model phone, but would like to know if anyone has any ideas how this might have happened.

Are you using any applications such as CSM or Xarios which may fire a rule to record calls?

No, nothing of the sort.

Sure sounds like you have an external app running, as IPInstaller suggested. Just to check, go to SYSTEM/SOCKETS and see if SYSTEM OAI LEVEL 2 is YES...if so, chances are there's an app somewhere. If not you can be sure it's not an external app.

Is this a 3300 and not a 5000?
The 3300 has some COS options that allow for automatic recording of all calls

It is a 5000 and there are no additional applications. Even my phone system vendor is stumped. I've removed the call recording application from his extension and we have a scheduled reboot this weekend. I'll put it back on Monday and see if the issue reoccurs. Thanks to everyone so far!

The AVDAP log may give you more information as to what is causing the record a call to happen as its voicemail related.
Take a look in their and see.
If not if you raised a ticket with Mitel with all the correct logs (including all the CPH files) they should be able to tell who or what is activating the record a call.
