Does anyone know why tiles turn gray in the MiCollab Console? I have a user saying that some of the user tiles become gray while the user is on the phone.

Based on the description, I would say the tiles are light blue in my case!

Button view: Displays all contacts as buttons in the Directory window. Button colors indicate the following:

White: Telephony status is unknown (for example, a personal contact with a PSTN number).

Light red: The contact's phone is in DND mode or forwarding calls to voice mail.

Light green: The contact's phone is ringing.

Light blue: The contact's phone is off-hook.

Light gray: The phone is unreachable.

So after lots of questions, it seems like the MiCollab server is losing the ability to monitor phones randomly. Anyone seen this before?

Yes!
Sometimes, MiTAI moniors seems to stop for some users.
Refresh line monitor cache in the UCA server. There's a small icon for that in the PBX tab. UCA server will then create a MiTAI monitor on the 3300 for each extension.
Also, I think there's an option to automatically refresh line monitors each night. Look in the PBX configuration in UCA server
