I think I have seen this before, but I can't remember where.
At any rate, I don't know how this happened, but at a customer's site, when extension 155 is picked up, it automatically dials extension 123. I have checked for forwarding as well as DEE and I see nothing.
We have tried defaulting the extension and resetting the system. Can anyone direct me to a solution?

Check the House Phone flag on ext 155, make sure it's set to No.
