Have a small 5000 system with just 3 POTS lines. The users are constantly complaining that when they make an outbound call, "half" the time the person they reach doesn't match the number they dialed or the number dialed that the phone shows.
Techs have made 20-30 test calls and been unable to repeat this, but later the same day the customer called back saying it just did it 3 times in a row.
I'm not sure the exact process that the end-users are using to make the outbound calls (either via ARS or by Trunk selection) but techs have tested both ways.
Anyone seen anything like this or have suggestions on how to diagnose?

I think this was an issue on V6 and was fixed in one of the Patch Release, what version are you running?

Probably one of the early public v6's.
I'll confirm that and double check.

Ended up opening a ticket with Mitel support.
They confirmed that this is a known issue. However, they don't have a patch yet. They "do have a fix coming out shortly for this issue" though.
Apparently it effects 53XX Ip phones, loop-start trunk, and direct trunk keys.
