Is it possible to route calls to different destinations based on the caller ID of the call?

Yes, if you use Call Routing Tables.

Create a CRT that is based on Outside Party Number.
You then add your Caller ID and destination.
Make sure the last entry is a + (Catch all wildcard) and the destination of another CRT that would normally route calls based on number dialed i.e. DDI.
You may also need to add "E" as an entry for calls that don't display CLI.
