Title: MAS Single Point Provisioning Error

I am trying to create a new user via MAS's single point provisioning (MCD 5.0 and MAS 4.0) and every time I get the following error

Failed to create DN: 2109 (lastname,firstname) on ICP: ABCcompany (10.1.2.20). (ICP Error: Entry contains out of range data.) The approximate system time on the ICP is: 14:56:11 on 2013/Feb/04.

Does anyone know what could be causing this error?
