A 5312 running version 04.01.01.07 is placing the call on hold when trying to retrieve voicemail.
When you call the voicemail number from the phone, the call starts and begins the greeting but then the handset goes dead and the line appearance starts blinking. If you push the line appearance button, the call resumes and you can access the voicemail. It only happens from one particular phone.
Does anyone know if this is this a feature I don't recognize that is set wrong or a malfunction?

5000, 3300/MCD or in SIP mode on something else?

Sounds like a sticky hold button.
Strange, though, that it doesn't happen when calling other phones.
How are you calling? Are you dialing the VM number manually or pressing a msg wtg button or something else?

I tried calling VM with the retrieval button, the speed dial and the direct number. All the same result.
Other calls can be made and answered without the problem.

That's really bizarre.
I can't think of anything that would cause that. - not even a defective phone.
Try swapping the phone out to see if the problem stays with the hardware or if it still happens with a different instrument.

A new day, a new situation. Now the problem is gone.

Did you replace the phone or did it go away all by itself?
Perhaps the PBX rebooted overnight.

I was about to replace the phone when the user told me it had cleared up by itself. I'm having her watch it and I'm going to do a reboot tonight.
