Using MiVoice Office 250.
Have a dynamic extension setup
Step 1) Goes to Desk 1 "an 5330" on site and Desk 2 "another 5330" off site at the same time, if no answer after 24 seconds
Step 2) Ring to VM.
Works okay for a while, then poof it breaks. Desk 2 requires removal and re-add. Works for a while and poof it breaks again. Its a vicious cycle.
Anyone seen this issue have any insight?

When you go to Associated Destinations under that User Profile do you see the non-working device as active?

I am not sure what you mean by "active", Its still showing up there at the moment. I am not the only one that works on this, so I will have to see if anyone has noticed.

Hmm, well if you go to where I explained to look then you should see if it is in the column field that shows, "Active".
System > Users > {User Profile} > Associated Destinations: you should see all of the devices and what their status is; see the attached example picture.

Yes, the destination that isn't working, shows as active.

It sounds as though the person is turning off their DEE either intentionally or unintentionally.
Look under System > Devices and Feature Codes > Feature Codes:
Dynamic Extension Express Off [Default: 362]
Dynamic Extension Express On [Default: 363]
Dynamic Extension Express On/Off [Default: 364]
If the first and last one are area/office codes in your area then most likely they are dialing without dialing the Outgoing Feature Code and turning on and off features.
You could add a digit to the feature code or force them to use the special key to use feature codes.
