The alarm has come up twice now in about a month. I found a post on another site and the suggestion was to delete the application and reprogram. Has anyone else had this problem and did delete/reprogram solve it?

1091? I am not familiar with that... VM alarms should be 020-039 or 200-224, can you double click the alarm in Sys Adm&Diag and grab a screenshot?

error Error number presented by the voice processing unit.
message Generic message generated by the voice processing unit.
CAUSE:
This message is provided to give Voice Processing a mechanism for outputting informational messages. Refer to the Voice Processing Diagnostics Manual (part no. 550.8019) Alarm 216.
ACTION:
Depends on the message.
FSACTION:
Report the problem to the engineering contact person for further analysis.

Thanks... I was able to find a copy in the 4.0 documentation set, release 11 from 2007. If you have something newer, sure.
