Title: call extension is diverted straight to operator?

As topic. Could this be a misconfig by user? Like set voicemail but no voice mail available? or maybe DND or something like?

Anyone any ideas. Has to be a site visit because we have no remote access but need to sort it tomorrow.

Any ideas where to look appreciated.
