Mitel MiVoice Business/MCD/3300 / Line or Call Swapping MiVoice Business 7.2

Has anyone come across issues the MiVoice Business v7.2 where 2 parties will swap calls?  We have been experiencing issues where employees are on the phone with people offsite via PRI calls and for no apparent reason the calls will swap.
