MiVoice Office 250/Mitel 5000 / Extension Conflict

I have a node that says extension conflict P#000 unassociated mail box. I can not seem to find anything related to that in the database programming. On top of that, the database repair utility says P#000 is a invalid extension and can not be repaired.

MiVoice Office 250/Mitel 5000 / MOH not working

I have a client that when the MOH device is plugged into the HX controller MOH port music does not play while on hold. I have checked at the trunk level that CP 5000 is selected as the MOH. I went to a phone and used the feature code 313 and no music came out of the phone.  I purchased an audio cord and plugged it into the 5000 and plugged the other end into my phone and turned music on. I went to a phone yet again and tried 313 feature code and no music played. I determined that the MOH port must be bad on the processor module. I switched out the processor module and did all the things mentioned above with the same result of no music on hold. I am looking for ideas on what else to try?
