MiVoice Office 250/Mitel 5000 / UC360 on MiVO250 sending double DTMF Tones to Auto Attendants

I have 2 UC360 that when it dials into a conference bridge and enter the access code the bridge returns entered digits 12347 as double i.e. "your access code 1 1 2 2 3 3 4 4 7 7  is incorrect" - I have this happening on 2 Different UC360 using PRI dialing into different services.

Any Ideas Where to Start
