Mitel 5000 - 5320 Phone flashing but not ringing

We have some users stating that they have seen there 5320 phones flashing so they pick up the handset a caller is on the line.
The phones flash but does not ring.  When the next call comes in the phone works correctly, flashing and ringing.  Our systems was updated to the latest firmware approx 4 weeks ago.  Any ideas on how to correct this?

Do you know how the calls that are not ringing the phone get there?
What does the display on the phone read before they pick up the call?

Is it possible someone is sending them calls using TRANSFER TO HOLD?  That sends the call without ringing, usually used when you want to "park" the call at an extension for longer than a regular transfer, which does ring but then goes to vmail (or wherever SYS FWD sends it).  Typically folks use TRANSFER TO HOLD and then page the user to alert them they have a call.
It's even possible users may have a TRANSFER TO HOLD button and not understand it.

I contacted the users and they stated that the calls were internal, extension to extension.  We will keep an eye out to see if I can get what is on the display when this happens again.

Any updates to this?  I have 1 user with the same problem.  Phone flashes but doesn't ring.  Just checking to see if you tracked the issue.

To get the system running again we restart all the Mitel servers.  After the reboot everything worked for a couple of months but then reappeared.  We have now updated the servers to 6.1 SP1.  Only time will tell if it comes back again.

We are having an intermittent issue that is occurring across multiple users with 5320 and 5330 handsets. When receiving calls they do not get any ringing from the phones speaker but the notification light is flashing and the call details are showing on the phone screen. The phones are idle when this is happening. It happens for external SIP calls to their DID numbers and for internal calls.
