MiCollab cannot make calls

We recently setup MiCollab so our employess can make phone calls through client software.

We have this setup in two locations that are working well.

Our third location cannot make calls or see active calls. 

In event viewer I see the following:
ucserver MiTaiMakeCall PBX_PROXY Warning  Wed 15 Oct 2014 12:08:11 EDT MakeCall by DN 1513 using monitor 11513 to destination 8842 failed. Reason: SXERR_MISSING_REQD_ATTRIBUTE.

Any thoughts what this error could be?  Doing a little research I cannot find anything regarding MiCollab, but I do see this error can be related to COS, but cannot be sure.

Any help would be greatly appreciated!

I had a very similar issue with a user that was hot-desking at a remote site in our cluster and here is what worked for me:
1. Deleted the extension from the MiCollab server
2. Deleted and recreated the extension on MCD
3. Reimported the PBX that the affected extension resided on.

After taking these steps, calls were successful on this extension. There are limited troubleshooting resources available and it took up quite a bit of my time; restarting MiCollab server, reinstalling the client app, etc...
