is there anything in the logs regarding packets being dropped? I would look maybe to enable voice quality auditing on the PBX to see if it shows anything.

Very odd.
Hopefully a reboot resolves it.   Let us know.
Another quick thing to try is renumber the phone - don't delete it- then reprogram it as a new one.

We may never know what caused this issue.  A reset server command fixed the problem but if it happens again, I'll try your suggestion of renumbering the phone.  Thanks!
