Yes the issue was the switch type.
The Mitel was orginally set to AT&T 5ESS, so I set the Adtran to the same.
I changed then Adtran to ISDN NII2 and then things worked properly.
When going to auto attendant, it would immediately answer and the Connect messages would start before the other data which when ringing an extension the Connect would be after this data. The messages were different, apparently the Mitel would not process things because of this.
It works with the Mitel still set at 5ESS, but changed that also to match.
As far as the noise - changed test phones and it went away. Start with simple things first.
